Pediatric Private Duty Nurse (LPN/RN) for 12-Year-Old Superman!
POSITION OVERVIEW
Looking for a kind, full-time LPN or RN in Monticello, Utah for my sweet 12-year-old nephew, Calel! (Calel is Superman's Kryptonian name.) He is incredibly kind to everyone he works with. This year-round position involves caring for Calel at home, at school, and out in the community.
COMPENSATION AND SCHEDULE
- Role: Full-Time Private Duty Nurse (LPN or RN)
- Pay: $23.00 to $34.00 per hour(We are asking the agency to set this up as a stable, salaried position)
- Benefits: Full benefits package to be provided by the hiring agency
- Sign-On Bonus: We offer a $2,000 cash sign-on bonus that can be used toward relocation expenses! This bonus can be paid out in full after completing the first 90 days of employment, or split with half on the first paycheck and the remaining half after 90 days.
- Schedule: Available Days: Monday through Saturday. 8:00 AM to 5:00 PM. You can chose your days you want to work. We will also hire a second nurse to cover the rest of the hours as this schedule is 54 hours a week.

- Upon approval, you can bring your children to the home, because Calel loves playing with and listening to other children as long as they do not yell, fight or hurt him or each other. We do ask that you fully clean up after your children before leaving the home at the end of your shift.
- Paid Time Off (PTO) will be utilized when Calel is gone out of town for medical or vacation, providing great stability for your income.
ACCOMMODATIONS AND DEVELOPMENTAL CONSIDERATIONS
Calel is a sweet 12-year-old boy who is cognitively at about a 7-year-old level. He is blind and uses a wheelchair for mobility. He relies on his care team to help him navigate and access his environment safely, and he does best with gentle, patient, and hands-on guidance throughout his daily routine.
RESPONSIBILITIES
- Personal Care: Perform straight cathing and diaper changes every 2 hours using our Hoyer lift or a 2-person transfer.
- Physical Care: Assist with transfers to a standing frame to support his physical health (2 times a day for a total of 2 hours), perform his daily leg stretches, work on occupational and physical therapy exercises, and closely monitor his temperature and other physical needs.
- Dietary Management: Oversee all meals and snacks to ensure he eats safely without choking. Maintain his strict dietary guidelines, ensuring everything remains completely dairy-free and refined sugar-free.
- School and Community Support: During the school year, you will meet him at school and work with him directly in the educational setting. Year-round, you will accompany or transport him to various activities using his 2023 Toyota Sienna wheelchair van, helping him engage with the world around him. This includes taking him on outings in the community.
- Travel for Medical Appointments: Occasionally accompany his care providers on medical trips to Salt Lake City to manage his care on the road.
REQUIREMENTS
- Active LPN or RN license
- Ability to provide hands-on care, including catheterization, diaper changes, transfers, and Hoyer lift support
- Ability to help with therapy exercises and monitor day-to-day physical needs
- Valid driver’s license and willingness to accompany or transport for school, activities, and occasional medical trips
- Willingness to work in the local area and cover scheduled shifts between Monday and Saturday, 8:00 AM to 5:00 PM
- Reliable, patient, detail-focused care style
PREFERED
- Pediatric nursing experience
- Home health or private duty experience
- Experience supporting a child with blindness or wheelchair use
- Experience with structured daily routines, PT/OT exercises, or school support
- Experience overseeing special diets or choking-risk meal support
